Predictors of Cervical Cancer Screening Uptake Among Women of Reproductive Age in Indonesia: A Nationwide Cross-Sectional Study Based on the 2023 Indonesian Health Survey

Abstract

Background: Cervical cancer is a major public health issue in Indonesia due to limited screening reach. Identifying the factors influencing screening uptake is critical for enhancing screening uptake strategies. Methods: This cross-sectional study utilised data from the 2023 Indonesian Health Survey (IHS) to explore the predictors of cervical cancer screening uptake among women aged 15-49 years. Bivariate and multivariate logistic regression analyses were performed to assess associations. Results: This study included 163,433 women, with only 8.7% (n = 14,184) reporting participation in cervical cancer screening. Age, education, occupation, health insurance ownership, information exposure, and parity were significantly associated with cervical cancer screening. Among these, exposure to cervical cancer information showed the strongest effect, with informed women having more than twice the odds of screening (OR = 2.252, 95% CI: 1.992–2.545). Conclusions: Cervical cancer screening uptake is low in Indonesia and is predicted by age, education, health insurance coverage, and exposure to cervical cancer information. Targeted interventions to improve these factors are essential for enhancing screening uptake and reducing the burden of cervical cancer. Future research should explore longitudinal studies to establish causal relationships between the identified factors and cervical cancer screening uptake.
